Intake – records the current family data composition such as family member names, address, social security numbers, annual gross income, level of education, etc. Eligibility for services is determined during this process.
Assessment – includes a joint effort of staff and client to determine client strengths, needs and barriers. The conclusion of a family assessment results in a referral plan to further address the client’s needs.
Referral – utilizes the information gained in the intake and assessment components to provide the client available agency services and/or existing community services to appropriately address the client’s situation.
Follow-Up – ensures adequate client progress. After referral, a follow-up of a client’s status will be made to determine whether the client has received the desired service(s). The object of this process is to ensure a “completed action” of all referred services.
